The Achilles tendon connects the calf muscles to the heel bone and takes enormous load every time you walk, run or climb stairs. Achilles tendinopathy is a common overuse condition causing pain, stiffness and sometimes mild swelling at the back of the heel, often worse first thing in the morning or after a period of rest, and during or after activity.
Who gets it
People who have suddenly increased their walking or exercise (common after Ramadan when Taraweeh walking increases activity levels), people who wear unsupportive flat sandals for long periods, those with tight calf muscles, and people returning to sport or exercise after a long break.
Symptoms
Aching or burning pain at the back of the heel, stiffness that is worse in the morning and eases with gentle movement, mild swelling along the tendon, and tenderness when the area is pressed or the calf is stretched.
Our treatment approach
Assessment of gait, footwear and activity pattern to identify the underlying cause; guidance on appropriate rest and gradual return to activity; advice on supportive footwear and, where needed, heel wedges or custom orthotic support to reduce tendon strain; and referral for further imaging or physiotherapy if the condition is severe or not improving with conservative care.
Prevention
Increase walking or exercise gradually rather than suddenly. Wear supportive footwear rather than flat, unsupportive sandals for long walks. Stretch calf muscles regularly, particularly before and after exercise or long walks.
